2026-2027 Permission to Tryout
This form must be used when a player from BADGHA wishes to attend a tryout hosted by another OWHA Association. This permission is issued by BADGHA. To ensure your Permission to Tryout ("PTT") Form is processed, you must complete all sections of this form. If you need more than one PTT Form, please submit a separate request for each one. Your request will be emailed to the BADGHA President for completion, so please ensure your email address is correct; this will be the communication method for sending your completed PTT Form. **Please note that incomplete information will result in delays in processing your request. Make sure all required data is included.** Requests will be processed as quickly as possible, but please allow up to 5 days for processing. Verbal requests for PTT Forms will not be accepted! If you are offered a spot on another OWHA Association's team, you will be required to register in RAMP with the new Association. This will initiate a player release that will automatically be sent to the BADGHA Registrar for approval and release.

OWHA PLAYER MOVEMENT/TRYOUT POLICY

Permission to Tryout ("PTT") Forms must be filled out and signed by a designated executive member of the player's current Association and presented to the Association for which the player wishes to tryout before she is allowed on the ice.  Failure to comply will result in a minimum two (2) game suspension for the offending player and/or head coach.

It is the responsibility of the Association hosting the tryout to obtain a copy of this PTT Form prior to the player participating in their tryouts.  It is the responsibility of the Player to have their PTT Form completed prior to the first tryout.  The PTT Form must be made available to the OWHA if requested and falsification of this form may result in a suspension of the player for up to three years as per Hockey Canada Regulations.

This PTT Form is NOT a player release.  All player releases are completed electronically via the RAMP registration platform. No player may participate in any OWHA sanctioned activity with a team other than where she was last registered, unless she has been released to OWHA Releases or have a properly executed PTT Form.

**Player Release Forms and PTT Forms are not required for movement to or from any Boys Hockey Association or for movement from a team not affiliated with the OWHA**
