The Belmont, Aylmer, Dorchester Girls Hockey Association (the "Association"), under the recommendations of the Canadian Hockey Association and the Ontario Women's Hockey Association, have adopted an online form for its award nominations.  This process precedes final confirmation and verification of statements made by the nominator.  Any information obtained  in this form will be disclosed within the Association and to the general public.

As a condition of submitting a nomination, all nominators agree to adhere to:
(a) providing a truthful nomination.
(b) providing information that in no way embarrasses the player and/or association.
(c) allow information to be shared with a committee that maybe outside the organization.
(d) Award winners information will be announced and shared to the general public.
(e) OWHA rules and regulations and code of conduct.
